Header error

After voting, I get this error at the top:

Warning: Cannot add header information - headers already sent by (output started at ...

I used a simple <? include('pollphp'); ?> to install it in a php file. What do I need to add to fix this problem?

never mind... I fixed it :)
Had to chop the file up.

Open poll.php with a text editor. In the file, you will see  and then near the end of the page, you'll see . Everything in between those, copy and paste it in a table on your main page. The part before , put it before the <html> on your main page.

how do you mean this?? What is poll.php ? Is this the name of the file where I see my poll ? I dont find this file? Please help me!
