I would like to insert a customized log in form.

A viewer reading the entries on 'index.php' chooses the link to 'addentry.php' would be interupted by my log in form and then be redirected to 'addentry.php'.

Where can I place my 'log-in' form. It is a php file.

You could just change the filename that the addentry link points to by editing config.inc.php
