I dunno if this is the right forum to post, i've been searching for help about my problem.

Need some help on how to create an HTML email. I can create a simple form and send it to the email but it is all plain text. What I want is after filling up the form with the user, all the data will be sent in HTML format.

I have this PDF form as a sample and I want to make this form in php and I need to make this form looks like the pdf when recieving the data.
