hi
first of all you must have a database if your database name is ex"gbook" your database user name ex"john" and your database pass ex"1234" so you configure your admin/config.inc.php file like this:

$GB_DB["dbName"] = "gbook";
$GB_DB["host"] = "localhost";
$GB_DB["user"] = "john";
$GB_DB["pass"] = "1234";

once you do it make sure your all files uploaded to server then type http://www.yourdomain.com/guestbook/install.php and press enter you will got the next step thier is 4 boxes fill it with same info as you put in admin/config.inc.php file and press the first one left button "Create Table" and follow Screen Instructions.Hope this will solve your problem.
I visit your site your main problem is not left align but your problem is Setting of table width as I study your guestbook pages source code I found table width 200 so i think thats the main problem.
Soloution:go to your guest book admin and then select style
from the Top menu now there you can see in the 2nd row table width its show 200 change it to 100% dont forget the % sign now from the bottom of the page click on "submit settings" now go back to your guest book and refresh the page.
hope you will find some thing good
I think the guest book is default set to left I did not understand correctly how do you want it,s to left please explain claearly and if possible post your site URL too.
find header.php file in your templates dir open it in one of your fav editor and past your code there

<STYLE type=text/css>BODY {
scrollbar-face-color: #000000;
scrollbar-highlight-color: #000000;
scrollbar-shadow-color: #666666;
scrollbar-3dlight-color: #444444;
scrollbar-arrow-color: #009900;
scrollbar-track-color: #000000;
scrollbar-darkshadow-color: #000000;
body {
</style>

hope this will solve your problem.
