I'm new here but I am having the hardest time getting a poll to work and I've tried everything - obviously I'm doing something wrong.
I don't have a lot of experience doing polls so thanks for helping in advance.

The first problem are the error notes at the top.

The second problem is when voting, the results do not come up but an error message instead.

What can I do to fix this? I'm on a time crunch and starting to stress about it!
