Guestbook v.1.5

i need help with my guestbook... i cant get it to work?
cause when i add an entry then submit it after i submit it i get this message: "Actually, we couldn't find the page you requested. Please check the URL. "

And When I Put The ! RIGHT URL ! i get this meassage:
"Please Check your Address"

can anyone help me out i cant get the my guestbook to work

wat do i need to do/get the things to make my guestbook to work
