Can't get ad poll 2.03 to work

i am new to PHP and i cant' get my poll to show up in my pages.
i want use a include tag to get it to show on my side menu but i can't get it working with with the following tag:

or the method that is displayed on the demo page.

Change extension to .shtml for the the SSI.
Or to .php for the PHP include.

Ok i think i am missing a step in the config of a new poll.
can i get a step by step on adding a new poll to my web site?
and i get a 404 when i try to access your site.

some1 else besides me had a dl link for my stuff for adv poll 203.
but you'll have to search the forums for it.
