Internal Server Error after installing Guestbook 1.5

I believe I've installed everything correctly...all of the paths are the same, where applicable, as on the previous guestbook I've been using. Any idea what's causing this error?

When the number of entries reaches it's max, I'm getting the same error others have received: "Cannot add entry to guestbook because the html tag was not found in some guestbook files." I am able to post to the guestbook up to my max number of entries per page. Anyone know how to fix this? Please email at webmaster@dtpmusic.com. Thanks.
