Once you click on submit, on the thank you for signing page most of the design is gone! It looks like it is loading the footer template but not the header.

The 'thank you' page does load with the footer on the page, but not with the header.... the solution.... copy the coding for your header and paste it at the top of the template for the 'thank you' page.
