adapting Guestbook 1.5 please help! :s

hi.. so i figured im not as clever or knowledgable as most of you here so i went for guestbook 1.5 (the perl one) rather than using the advanced guestbook. and i got it to work fine which is fantastic!
however, i then went on to try to create a second guestbook of sorts, by adapting the original code and using different names for the script. but it doesnt work. any ideas?
http://brownie.pressurised.net/help/musicform.txt is the link for code i adapted.
the base url and directories are correct.
i have created the guestbook_file.html, id_count.txt (with a 1), page_count.txt, and sample.html files.
the addentry page (http://brownie.pressurised.net/addmusic.html) is exactly the same as my original guestbook one, all except the form tag has action="musicform.pl" instead of "guestbook.pl"

everything seems to be correct... but it still doesnt work!! can anybody help?
please someone reply even if its to tell me i cant do this at all lol.
thanks in advance
james
