What files/folders do I upload and in what folder structure?

Do I unzip pollphp into a pollphp folder and then upload it to my domain or should I create a folder on my host and put the files in it?

I have installed AP before and it worked except it didn´t show any pictures/icons (some path problem I think) and somehow I fucked it all up and am starting over.
