I will want to use the database version, but what files go in what folder by extension type ?

Are some files in this package not used for the database version?

I downloaded "pollphp.zip" from this website.

Once I get rolling, a little, I think I will be OK

We have this tree to fill which I understand (all except (etc.) ....
I just don't know what goes where in the 4 lower folders...

public_html
poll

admin
image
include
(etc.)

I have searched... TIA....
