Poll was working but now gives errors

My poll app was working fine for a long long time. Then I updated my website. 99% of the changes were just design and using more CSS in the style but no content changes. The code on my homepage that included the poll did not change at all and I didn't even touch the directory where the poll app was installed. Now when I vote, it works but it doesn't prevent multiple votes and I get the folling error at the top of my page:
