Can't get passed Advanced Guestbook Setup

I need to know how to get passed the Advanced Guestbook Setup page!

What do I enter at YOUR DATABSE?
And what do I enter at MySQL Host?

I always get following error:

Database: ask your server admin
host: in most cases it's 'localhost'
