Double check the paths and chmod's.

You are correct in your assumption about the locking timeout.
Just set it to something like 999 or for a long time.

If you have it set to ip Table you don't need the to put in the poll_cookie.php include code.

Probably like you said a cache problem or cookie problem (I think).

Good idea. Not likely to happen. No webmaster / forum admin.

There is a default, it should be in the zip file zomewhere.
After you get in make sure you go to General Settings and change the poll
path to what it should be and then click save settings or whatever it says.

Also change the username and password.

Yes, in the admin section.
The poll templates i used were fairly basic with most of the HTML removed and replaced with my own and CSS classes and id's.

Btw, i am Zeon, another one of my alias' lol.

The guide/manual with Zeon in is over a year old now and the other is a lot less than that, about 9+ months.
