okay on that page in my sig there is a link saying advanced poll documentation.
click on it then click on the download like and wait for the download to begin. Simple.

admin panel -> click on link at top furthest to right ->
3 text boxes will have the code for newest, random and a chosen poll/s.

also blank out your MySQL username and password with '*' - VERY IMPORTANT SECURITY ISSUE!

Are you doing the editing for the correct template set?
Otherwise your browser is on the friz or you've screwed something up
or adv. poll has gone majorly wrong somewhere. Apart from that I can't
help you any further. And I wouldn't expect much help from anyone else
but me. Well good luck in solving your problem!

complain to the original author about crap support (he's only posted about 25 times).

go to poll admin area and click on the menu icon furthest to the right.
this page shows you all the code for different polls eg. newest ect.
this page also shows you the full path to booth.php this path is the same for poll_cookie.php.
so try using that path instead.

Well that kinda sez it all.

Warning (as in problem here!)
fopen (something done to a file)
failed to open (something done to file didn't work)
permission denied (your not allowed access to it)

Answer... (most likely)

wrong chmod permissions.
