Going by the error messages maybe you got the permissions wrong or the image upload folder' path wrong?
You may find this site useful...

www.htmlgoodies.com
To find the bit of code above go to the templates section in the admin panel.
u need to fill in ur db details in the config file.

by the looks of things ur db DOES REQUIRE a password.
admin panel and mysql do NOT use the same password btw.
ah, my bad, i meant yes the code u posted a couple of posts above will work but it does not need the http://... bit. I meant you could use that instead of the one the poll admin panel gives.

sorry for not explaining it more clearly.

- both pieces of code you posted WILL work.
nope, it goes from your site's root directory.

you could use http://www.yoursite.com/poll/booth.php if you wanted.
but its easier to use the one it gives you.
Have a look at the demo on this site or go to your poll's admin panel then the last option on the menu at the top (far right) you should get some example code to use with the full path to the poll files already filled in.

K, but still, going by the error message the file either doesn't exist or the path is incorrect.
you have a | in you path.
Please read the sticky as I believe this is covered in there.
admin -> templates -> edit how you wish

I'll try and remember that in future

Do a search in all the poll files for: $this->form_forward.
Then post the code of that section before modifying it.
There should be a topic on this already.

But if the page it should return is the same all the time you could try setting it as the default path in the admin panel. Although there is probably some way to code it into one of the files i.e. add it to the header refresh if adv. poll uses those to redirect the user after they vote.
