I assume your customisations are only theme based. The only changes to templates I am aware of are to add $CAPTCHA variable to form.php and add the actual captcha template.
Looks like you didn't upload all the files.
And you posted the same message twice because? The error message says it all

Access denied for user: 'root@localhost' to database 'guestbook'

Put your database info into the config.inc.php file.
You can use www.dnstools.com to find out who owns the IP block and then report the spammer to them but a lot of times the spam is from a worm/trojan/virus on a persons computer.

Personally I would say CAPTCHA is better because some spammers spam by using a program to post the entries into their web browser using API.
If you have the ip then add it to the banned list and turn on ip banning.
phpMyAdmin is probably your only fast option.
I suggest updating to 2.4.1 and turning on password protection for comments.

If you want to keep the IP logged message.
Look in the template called entry.php. You will see the relevant variable in there.
Post the relevant lines in here with about 5 lines from before and 5 from after and I will show you how to fix it. The other alternative is editting the templates.
It's somewhere in gb.class.php but off the top of my head I can't remember where.
Put it at the top of form.php
