I was waiting upon Auron to answer this. Did you include all the files you are supposed to include?

You don't need PHP to create the form just to process the submitted data. You also need to decided if you are going to send the data using GET or POST. Get puts the data into the url whilst POST sends it differently. Also how are you intending to store the data?

Strange. This guy does use the guestbook but my forum was getting spammed the same as this one and all the members that were posting the spam had used @yandex.ru email addresses.

The options to send emails can be found in the General Settings section of admin.

The apache log should always log the ip and not the hostname. Very weird. I did manage to get moderndedicate.com resolves to 65.254.51.100. If you wanted to you could try reporting the spam to one of the suport staff listed on http://www.moderndedicate.com/en/

I'm guessing you are using the text version. It looks like the write permission to the data files has been changed. I believe they should be 777.

I think main(./poll_cookie.php): failed to open stream: No such file or directory explains it simply enough. Check the path you put in the include call.

I believe you set the colours when making the poll.

Hm I'd suggest speaking to your host about the to many connections issue. I get it a lot with an old site I no longer maintain and thats down to terrible set up of the server.
