You're right, it's working now!
Eh... now the guestbook gives me a warning:

Carbonize wrote:So long as the box is ticked and the password has been supplied it should appear. Maybe you are missing a template such as com_pass.php.

com_pass.php is in the templates directory.
Very strange, isn't it. I hope you can help me.
Carbonize, you have no idea?
Carbonize wrote:When you ticked the box in General Settings did you also provide a password in the box?

Yes, the normal password: comment
Here a screenshot of the comment page, so you can see there's no box to fill in the password (which is required):

Hello,

I get the following when a comment is made:

Warning: Cannot modify header information - headers already sent by (output started at /home/patrick/domains/patrickneumann.nl/public_html/gastenboek/comment.php:6) in /home/patrick/domains/patrickneumann.nl/public_html/gastenboek/lib/comment.class.php on line 219
No not that! That box I see, but when I want to make a comment, there's no box shown for the password and it says that my password is false...
I filled in that a password is required for make a comment, but there's no box shown where I can put in this password if I want to make a comment.

Can anybody help me and tell in which file I should be?

That'll be great!
Are you gonna repair this in a next version, Carbonize?
Carbonize wrote:Whoops my mistake. Change the 755 to 0755. But to be honest 644 should still be viewable.

It works again.
644
Carbonize wrote:Try changing the 755 to 777 in the code I gave you.

No warning, but still no image.
