To which changes are you refering?
Three layers yes. Or could be classed as four.

1 - The submit button's name is slightly different on every version of Lazarus.

2 - There is a time delay before it will let a guest submit their entry (optional)

3 - There is a time limit for the guest to submit their entry in (optional)

4 - Human verification where the guest has to answer a question before their post is accepted (optional)

Five if you include the ability to require posts to be approved before they appear in the guestbook.
The message indicates that the supplied database information is wrong. Speak to your hosts about the correct information.
Check the database information you provided in the config.inc.php file.
Personally I say remove the inage verification by replacing all the guestbook files except config.inc.php then try the human verification method.
The time/date is stored as a unix timestamp. A unix timestamp is the number of seconds since it turned midnight on January 1st 1970 if I remember correctly. There are many sites out there that wil convert a date into a timestamp for you.

Yes update to 2.3.4. You should always update to the latest version of a program or script before making bug reports because most of the times the update has fixed them.

Delete the addentry.php file and remove any links to it from the guestbook.

Yes the IP ban link is great so long as you remember to actually turn on Ip banning unlike me. I spent 30 minutes looking for a bug that didn't exist.
