he really needs to rewrite his install script.
If you didn't edit the reset.php file then the username and password should of reset to the defaults of test and 123 respectively

Weird as changing the extension should make no difference to how it is rendered (except for the php working that is)
survey1.htm you need to change the .htm to .php
It is possible I'm just to tired to think of it right now. You'd basically have to merge the thumbnail script into the guestbook and turn the result into a variable to be processed as part of the template.
I'm guessing you mean Javascript in which case yes. You just need to copy the code into the relevant template file(s).
You are welcome to test Lazarus for me, just email me and I will reply with a copy. Remember it is still beta so may contain a few bugs.
You needn't use an include with the admin.php file because when all is said and done it's not something the public is meant to be looking at so having it fit into the site is not that important
The best way of integrating the guestbook into your site is to edit the template files header.php and footer.php. I have discussed this elsewhere in the forum and I'm pretty sure there is a link to the relevant thread somewhere in the sticky. Basically you copy all the HTML that wil appear before the guestbook into header.php, remembering to keep the CSS and Javascript. Then all the HTML that will appear after the guestbook goes in the footer.php file.
The only issue with using AGcodes is spammers using it to post hyperlinks.
Read the sticky thread. There is a link in there to a simple anti spam solution that will stop some of the spam.
I'd say that yes you updated from 2.2. 2.2 retrieved the data differently. The reason your guestbook is running slow now is the high number of comments you have in some of the entries. Resolving this issue is on my list of things to do and i do have an idea which I may work on later tonight.
Speak to your hosts. The problem is with the server not the guestbook.
Is this since upgrading from 2.3.1 to 2.3.2? or from 2.2 to 2.3.2 ?
