So long as the page/s that you want your poll to appear on have the extension .php then you and fine and ready to go!
No extra steps are required to get it to work or display. : )

Find the page you want your poll on.
Eg. index.php - must be .php unless you want to mess with .htaccess files.
Then add the code from the admin panel to the page, placing it where you want the poll to appear.
Eg.

That's the hostname ie. info about where the poster came from usually used with a IP Address.
You'd have to get the e-mail addy and add it to the e-mail when the post gets processed by the PHP script.
Note - i may be talking rubbish atm ; )

A .htaccess file could solve the problem.
It depends if your web server has been configured to look for them.
Basically the .htaccess file will tell the web server to treat the .htm/html file as if it were a .php one.

Try leaving the host field as localhost since that is the default.
If localhost does not work consult your hosting service provider.

Can't help too much on the first problem.
All I can say is what's the point in having a host with PHP but not being allowed to use the .php extension. It's just plain stupid.
The poll_cookie.php file MUST BE AT THE TOP of the file its included in.
Eg. BEFORE <html><head> .... so something like this...

If you can change the page extension to .php
That will stop all the poll_process() stuff appearing.
If you can't and you know your host HAS php then you can add a few lines to an .htaccess file which will or should tell the web server to process .html files as if they were .php ie. send them to the php interpreter.

If you have phpMyAdmin access you can find out your username.
To get your password reset get the default one from a new install since it'll be encoded.
Not sure if this will work but you could try and put in a temp password and then change it once you get in.

The cookie expires after 96 hours IIRC.
Open up poll_cookie.php, its one of the first few lines in the file.
