One of the benefits of using Firefox as your web browser is that it's easy to disable Javascript and with Webdeveloper extension just as easy to disable meta refresh.
Just to confirm we are talking about Advanced Guestbook 2.something yes?

In the left pane, where it says 'No tables found in database' you would usually get a list up od available databases. Or if you only have one database a list of the tables in that database.
You need to select the database for your Advanced Guestbook and then click book_data in the left frame. This will open up book_data in the right frame. Select Browse at the top of the right frame and from here you will be able to delete them.
It's something your host usually provides. As I said before you are welcome to supply me with your details and I shall delete the offending entry.
Surprised you're using AG for this though as would of thought a more basic guestbook would be more suitable.
BTW for security reasons disable HTML.

Well what you ask is simple enough even using Javascript to make the new inputs appear if they say they are an old guest. Just reluctant about removing the name being required is all.

Have to love clients and their stupid requests.
Anything is possible although I'm not sure why you would not want the name field. Personally I feel that is important.
It is allowed to ask for anything you wish so long as it is related to the guestbook. What exactly is it you are after in the way of customisation?
PM me with your guestbooks url, your username and password and I will delete it for you. Or if you have phpMyAdmin you can delete it that way.
Did you set the permissions on both the public and tmp folder to 777 ?
Turn it off in General Settings I think. Or just remove it from the form.
