No you simply import th SQL file to the new server using phpMyAdmin, edit the config.inc.php file to reflect the new database settings then upload the files and that should be it.

To transfer the database you will need phpMyAdmin.

By older version what do you mean? Guestbook 1.5 or 2.2?

Can't see why it shouldn't be working. Might be a newer version but the tables should be laid out the same way.

Is is recording the comments but not the images.

At a guess it means your host is running in safe mode and this is preventing the guestbook from running certain commands.

Are you sure you are using the Advanced Guestbook from this site as you are getting error messages about files called extension.inc and a flder called common which are not part of Advanced Guestbook 2.3.x. They may be part of Advanced Guestbook 1.5 which is written in Perl and does not use a database but I cannot download it to check as my works have blocked the downloading of zip files.

Odds are the spam entries are manually entered and there is little we can do about idiots like that.
